How to live in an SDA Home
Participant Process
ADAPT are working with participants, family members, SIL Providers and Support Coordinators to provide housing solutions across the country. We identify participant needs through the following process.
1. Talk to ADAPT about your housing needs
Talk to ADAPT about your housing needs and fill in a Participant Expression of Interest form. This will provide us detailed information about the partcipant, their current living situation and what suitable home environment they wish to live in the future.
2. Lodge your housing solution report
After asking for EHOP (Explore Housing Options Package) funding from your NDIS Planner, ADAPT or your support coordinator can find a suitable Report Writer to submit your SDA Application to the SDA Committee.
Search for an available home or build one.
While waiting for your SDA Approval, or after it is granted, you can work with ADAPT to find or build a new SDA Home for you to live in once your SDA is approved.
Move in to your New SDA Home.
Once your SDA Approval is granted and you sign an accommodation agreement with ADAPT we help prepare you to move-in and decide the team that will be supporting you in your new SDA Home.
